My wife's Pleco system on her iPhone now has scrambled entries after she updated her IOS from 5.0 to 7. The whole system is garbage now: characters have the wrong definitions. Is there any way to fix this? I don't think she had a back up unless it is done automatically on the cloud.

Update her to the latest version of Pleco via the App Store "Updates" tab - she's using an extremely outdated version that's not compatible with iOS 7.

She did the update and said it looked like the update went through, but the listed number of needed updates didn't change and Pleco still has the same problem. Does she need to uninstall and reinstall? Will her flashcards be lost in the process?

Yes, in that case she should back up her flashcard database (use the "Backup Database" command in Flashcards, then send the file to herself as an email attachment through File Manager) and then delete and reinstall.

Working now. Thanks. Strange, after using the file manager to email the files, the next morning Pleco was suddenly working fine without having uninstalled and reinstalled.
